A 12-lead electrocardiogram represents a long QT syndrome in in a 49-year-old male on therapeutic dose of methadone with a serum potassium level of 3.1 mmol/L and no cardiovascular disease history seen in the patient’s family. A burst of TdP can be seen on the left, and short time (under 10 s) tachycardia can be seen on V1-V6 (right). The electrocardiogram is used with the permission of Dr. Pierre Taboulet [32]
